Objective To explore the application effect of health education based on knowledge-attitude-practice model combined with multidimensional collaborative continuity intervention in end stage renal disease (ESRD) patients undergoing hemodialysis.
Methods Eighty patients with ESRD undergoing hemodialysis were selected and randomly divided into a control group and an observation group, with 40 patients in each group. The control group received health education based on knowledge-attitude-practice model, while the observation group received multidimensional collaborative continuity intervention on the basis of the control group. Both groups were intervened for 2 months. The self-management ability, healthy behavior, hemodialysis complications, and quality of life were compared between the two groups before and after intervention.
Results After intervention, the self-management ability, healthy behavior, quality of life in the two groups were obviously improved compared with before intervention (P < 0.01), and the above indicators, incidence of hemodialysis complications in the observation group were significantly better than those in the control group (P < 0.05 to P < 0.01).
Conclusions The combination of health education based on knowledge-attitude-practice model and multidimensional collaborative continuity intervention can enhance the nursing outcomes for patients with ESRD undergoing hemodialysis. This approach positively impacts patients' self-management ability and compliance, helps reducing complications, and improves patients' quality of life and satisfaction with nursing care.
