Construction and application of Information-based ICU-rehabilitation transitional care program for critically ill patients

Abstract
Objective: To construct an information-based ICU-rehabilitation transitional care program for critically ill patients and explore its application effect. Methods: A total of 102 critically ill patients who were transferred from ICU to general ward were selected as the study subjects,and divided into a control group and an observation group,with 51 cases in each group.The control group implemented conventional transfer care,and the observation group was given the information-based ICU-rehabilitation transitional care program on the basis of the control group.The length of hospitalization,anxiety level of patients,and relocation stress level of family members were compared between the two groups. Results: The length of hospitalization and readmission rate of patients in the observation group were lower than those in the control group (P<0.05).Three days after ICU transfer,the anxiety level of patients,and score of relocation stress level of family members in the observation group were significantly lower than those in the control group (P<0.01). Conclusions: The information-based ICU-rehabilitation transitional care program can optimize the continuity and coordination of health care for critically ill patient,reduce anxiety after transfer,and help to shorten the length of hospitalization and reduce the relocation stress level of family members.
